Design the Surveys & Contact Forms per Referral Page

Each project can have several external referral pages (Public Forms)

  • It is a web page that you can share with your referral partners

  • They can use it to send potential clients/service-users/beneficiaries/contacts to your project’s Waiting List.

 

The Referral Page contains

  • 1 Contact Form: Mandatory

  • 1 or more Surveys: Optional (you can choose to have no surveys)

 

On the Surveys & Contact Forms per Referral Page tab of the Blueprints, add the

  • Contact Forms that will be in one or more of your Referral Pages to column A

  • Surveys that will be in one or more of your Referral Pages to column A

  • Names of your Referral Pages to columns C onwards in Row 3

  • Mark a Yes to indicate which Contact Forms and Surveys will be used on which Referral Pages
