Create a Signup Page that includes a Survey

You can create a 2-page Public Form that includes a survey

  • Page 1 is the Contact Form that creates a new contact

  • Page 2 is the Complete Survey Page that allows the newly created contact to complete the survey (i.e. create a story)

Steps

  1. Select My Apps in the top menu

  2. Select Contacts

  3. You arrive on the Manage Contacts page

  4. Select the More Options button in the Contacts App

  5. A dropdown menu appears

  6. Select Signup Pages in the dropdown menu (formerly called Referral Forms)

  7. You arrive on Signup Pages

  8. Press Create Signup Page

  9. You arrive on the page to create a New Signup Page

    1. Give the Signup Page a name

    2. Choose the project you want contacts to be created within

    3. Choose the state (Waiting List, Enrolled, Alumni, Declined) that you want contacts to be added to

    4. Choose the Contact Form you want to be completed

    5. In the Select one or more surveys field, choose the survey that you want to appear on Page 2

    6. If you select multiple surveys, the form will automatically add additional pages; one page for each survey.

  10. Press save

  11. You return to your Signup Pages. There is now a web link for your new Signup Page

  12. Copy the link and share it with your referral partners or display it on your website
