The Tracker-based questions you plan to add to your Survey Template must exist before you can add them to the Survey Template. To create new Tracker-based Questions, read Create a Progress Tracker

There are two types of question that can be added to a survey

  • Tracker-based questions

  • Field-based questions

To see the difference between the two, read Add questions to a survey

Steps

  1. Select My Apps in the top menu

  2. Select Surveys

  3. Use the Manage Surveys For toggle to go to the library containing the survey you want to edit.

  4. If the survey is owned by your organisation, select the name of your organisation in the dropdown list

  5. The title of the page changes to reflect the library you are now in

  6. Switch to List view

  7. Use the page numbers at the bottom of the page to find the survey you want to add questions to

  8. Click the name of that survey, shown in blue

  9. You arrive on that survey’s template page

  10. Click More Options in the header menu of the survey and select Edit

  11. You arrive on the Edit survey template page

  12. On the left column are two panels

    1. Your Questions Library

    2. Public Questions Library

  13. Your Questions Library lists the Progress Trackers that you have created and/or which are owned by your organisation. Click Add to add one of these to the survey template

  14. Public Questions Library lists all the Progress Trackers that are available within the platform for you to add to your survey. Click Add to add one of these to the survey template

  15. Progress Trackers of the Indicator-type (Achievement, Multiple Choice, Numerical) are nested underneath an Outcome. To add all the Progress Trackers within that Outcome at once, press Add next to the name of the Outcome

  16. When you add a question it appears in the right column that shows the Questions in the survey template

  17. Use the other options in the survey editor to

    1. reorder the questions,

    2. set questions as mandatory,

    3. add points to questions

    4. set the Multiple Choice Type for multiple choice questions - as single, limited or unlimited

    5. (To add skip logic, read the guidance.)

  18. Scroll to the bottom and press Save
