  • When a survey is loaded as a form for completion, the Field-based questions only appear once rather than for each contact.

  • When you want Field-based questions to appear for each contact that is in the story, enable that option on the Survey template page

How to enable Fields For Each Contact

You must have permission to edit a survey in order to enable Fields For Each Contact

  1. To enable fields for each contact, go to the Survey template page.

    1. If you are unsure how to get there, read How to access the Survey Template page

  2. On the Survey template page, tick the Manage Fields tick box

  3. A green Manage Field button appears next to every Field-based Question.

    1. It will not appear next to the Progress Tracker based Questions.

  4. Click the Manage Field button that appears next to the field that you want to switch from being Once per Story to Once for Each Contact

  5. A popup appears in which you can choose the mode for that field

  6. Make your selection and press Save

  7. A confirmation popup appears and the page reloads

  8. Press the Manage Fields button again to repeat the process for each field within the survey that you want to adjust the mode for.

  9. That’s it.

    1. When you use the survey (whether on a project or as an Event Attendance form), the fields set to Once for Each Contact will appear multiple times - once for each contact.
