In a Contact Form you can change the way that a multiple choice question is displayed. You can choose between
Dropdown Menu
Expanded Set of Buttons
To do this, you just need to use the Layout toggle to change that question’s appearance.

Steps
Go to the My Apps option in the top menu.
Click on Contact Forms.
You arrive on the Manage Contact Form page. Manage your Contact Forms
On the Manage Contact Form page, click on the title of the Contact Form.
You arrive on the Contact Form’s profile page.
Scroll down to the Multiple Choice (Dropdown) question.
Click on the Actions Button
Click on Layout option.
A popup appears with choices of Compact and Expanded for the options.
Select the option for how you want to display the options for this particular question.
You can set the layout for all the questions similarly.
