Before you can create a Referral Link you must turn on Referral Links in your organisation: Turn on Referral Links

Combined Referral Pages allow you to capture information about the referrer, the referred client and other people related to them

How you might use it

  • you want to have a referral form that captures details of three entities:

    • The client who is being referred to your service

    • Their next of kin, e.g. their father, sister, etc

    • The referrer who is actually completing the form

  • In this scenario the client would be Person A, their next of kin would be Person B and the referrer would be Person C.

Steps

  1. Go to the Create/Edit Referral Link page as usual

  2. Click the Add Another Person button - this lets you set add a contact form and set of surveys for another person within the referral journey

  3. Indicate the relationship between Person A and Person B

  4. Indicate which person should be saved in the system as the referrer

  5. Press save
