Public Forms (Referral Pages) are pages that anyone can access (without needing a Makerble Account) and use to create one or more contacts within one or more of your projects.

How it’s used

  • Self Referral Forms: allowing people to refer themselves to one of your projects

  • Professional Referral Forms: allowing referral partners who work with vulnerable people to refer someone to your services

  • Referral Surveys: allowing people to complete survey which contain demographic data as well as feedback and/or impact data

  • Self Registration Forms for New People who turn up at an Event: allowing ‘walk-ins’ to give you their details and log their attendance when they attend one of your events for the first time

How it works

  1. DESIGN: You start by creating a Referral Page.

    1. You choose which project the new referrals should be added to

    2. And you choose which ‘state’ of that project they are added to, i.e. whether they are added to the Waiting List or the Enrolled list. (You could optionally add them directly to the Alumni or Declined state if you really wanted to)

    3. You choose whether there are any baseline surveys that need to be completed as part of the referral process

    4. You choose whether the Referral Page allows for several contacts to be created at once who are linked together using Relationships. E.g.

      1. Page 1 could create a contact profile for a child that needs your services,

      2. Page 2 could create a contact profile for the child’s parent

      3. and Page 3 could create a contact record for the Professional Referrer who is referring the Child to your service.

  2. SHARE: You share the Referral Page website link.

    1. Anyone with access to the link can use it to create new referrals (i.e. create new contacts)

    2. People do not need to have a Makerble Account to use your Referral Page

    3. You can deactivate a Referral Page if you no longer want it to be used to generate new referrals

  3. RECEIVE

    1. All new referrals (contacts) are saved within Makerble and are accessible via the Contacts App

    2. You can turn on Referral Notifications in order to receive a web notification and an email every time one of your projects receives a new referral

  4. FIND

    1. You can use the Contact Filters to quickly find your new referrals.

    2. The Contact Filters allow you to display, for example, all the contacts created within a particular Date Range who were generated through the Referral Form and who are still on your project’s Waiting List.
