  1. Select My Apps in the top menu

  2. Select Contacts (if you don't see Contacts as an option, you can access the Contacts App via the Control Panel)

  3. You arrive on the Manage Contacts page

  4. Select the More Options button in the Contacts App

  5. A dropdown menu appears

  6. Select Public Forms Forms in the dropdown menu (formerly called Referral Forms)

  7. You arrive on the Public Forms page

  8. Press Create Public Form

  9. You arrive on the New Public Form page

    1. Give the link a name

    2. Choose the project you want contacts to be created within

    3. Choose the state (Waiting List, Enrolled, Alumni, Declined) that you want contacts to be added to

    4. Choose the Contact Form you want to be completed

    5. Optionally choose Surveys you want to be completed

  10. Press save

  11. You return to the Public Forms page where there is now a web link for your new referral page

  12. Copy the link and share it with your referral partners or display it on your website
