You can create a question while creating a survey
Steps
Select My Apps in the top menu
Select Surveys
You arrive in the Surveys App
Select Make a Survey
You arrive on the Survey Builder (Create Survey page)
Click New Question or section button to create a question
This launches a popup that lets you create
a quantitative question - i.e. a Multiple Choice, Numerical or Tickbox question
The Outcome field might be set to your organisation’s automatically-generated 1st outcome by default
If your organisation has created outcomes, you can use the dropdown list to select the one you want to link to this survey question

Section Blocks
When you press Save the popup closes and the newly created question is added to the bottom of your survey questions listed on the right side
Once you are done, scroll to the bottom and press Save
